First Appointment Information

Location

• The address is: 1195 Chesapeake Ave. Grandview, OH 43212

• Please park in one of from spots labeled “Clients of 1195”.

• Enter through the side door located in the alley to the left of parking. You will see a waiting area with chairs.

• If you walk all the way around to The Human Form you have gone too far.

• If you arrive early, please relax in the chairs provided in the hallway while you wait for your therapist to welcome you into the treatment room.

• The session will be 55-60 min long.

• In the event you are late to your session please understand that our session will still need to end at its scheduled time.

late Cancellation/no show policy

• Please note that cancellations, rescheduling, or missed appointments (no-shows) require at least 24 hours' notice.
  Sessions canceled with less than 24 hours’ notice—or missed without notice—will be charged in full. This applies to both pre-purchased sessions and individually billed sessions at $125.
